Welcome to the website of the New York State Care Management Coalition. The Coalition’s goal is to offer care managers across New York State the opportunity to become one voice on many issues facing both the clientele and the agencies we all serve. In addition, the Coalition strives to provide quality, state-of-the art educational programming, information and networking opportunities on best practices in the care management field for the ever expanding group of professions which provide care management.
Our membership has grown in numbers and the interest in our annual conference is growing each year. Last year we accommodated over 700 people to our annual conference in Albany and this year we are moving the conference to Niagara Falls. We fully expect that this year we will solidify our conference as the largest behavioral health conference in NYS. Health Homes, DSRIP initiatives, integrated care projects, all point to a need for care management. In the many committees and sub committees I am part of, the common denominator in all the new initiatives seems to be care management, care coordination and boots on the ground assistance. These are the interventions that move the needle and get people to the outcomes they hope for.
The statewide Board of Directors hopes that you will find this site as a resource to you and your colleagues by providing information on our Annual Conference and other care management issues. Please take a look at our board listing. The Coalition’s Board members in your region are willing to assist you both personally and professionally regarding New York’s care management system. Also, the Coalition’s Board of Directors meets quarterly with representatives from the NYS Office of Mental Health to discuss policy, legislative and regulatory issues that may impact care management providers and the people we serve.

In addition to meeting with OMH and DOH, the Board meets monthly via conference call and/or in Albany to stay in tuned to the ever changing environment we are all part of.
